Is my care provider set up for openPASS?

Your care provider must be using The PASSsystem and be set up for openPASS to get access.

If they are, you will see a unique ID number on the front page of the care receiver’s care file, which is printed in the home (see below).

If this is not present, fill in the form below and we will reach out to them directly to see if they would like to start using The PASSsystem.
